The practice undertakes a variety of health promotion activities, including:
The practice has a strict policy regarding confidentiality and data protection. We will only release test results to the person to whom they relate unless that person has given prior permission or they are not capable of understanding the results.
When you attend for tests that have been requested by any member of the practice team, the phlebotomist will advise you to contact the surgery 10 days after to obtain the results. The reception team can give you results - only to say if they are normal or if there is any other instruction on the result by a doctor or nurse. They CANNOT advise on specific results.
For all tests undertaken on behalf of the hospital, the results will be advised by your consultant or a member of his/her team.
Results are normally received within 10 days. Please ring after 11:00am for your results.
Cervical smear results are usually received within 4-6 weeks, and you are automatically notified of your result.
Planning your care in advance involves a process of discussion between you and the people who provide care for you eg. your doctors, nurses, social worker, family and friends.
Taking part in this process gives you the opportunity to express some views, preferences and wishes about your future care so that they can be taken into account if you were unable to make your own decisions at some point in the future.
The advance are planning process involves:

4. refusing specific treatment
5. Identifying who you would like to be consulted on your behalf
6. Letting people know your wishes

You do not require a doctor's sickness certificate for any illness lasting seven days or less. Your employer may however require you to complete a self-certification form (SC2) which is available from your employer or on the HMRC website.
If you are sick for more than seven days, your employer can ask you to give them some form of medical evidence to support payment of SSP (statutory sick pay).
Your employer can ask you to confirm that you've been ill.You can do this by filling in a form yourself when you return to work. This is called self-certification.
If you're sick and off work for more than seven days, your employer will probably ask for proof of your illness. Most employers ask for a fit note from your GP.
However, this will also depend on your employer's company policy on sick leave (or sickness absence). This policy should tell you how many days you can be off sick before you need to provide proof of illness or a fit note.
You could also provide evidence from someone who is not a medical practitioner, e.g. a dentist. Your employer will decide whether or not this evidence is acceptable. If your employer has any doubts, they may still ask for a medical certificate from your GP.
The 'fit note' was introduced on 6 April 2010. With your employer's support, the note will help you return to work sooner by providing more information about the effects of your illness or injury.

Carers Information Pack
Most carers don't see themselves as carers - they see themselves as someone who's looking after a relative or a friend. Carers can be of any age and don't just look after someone who's elderly - they could be caring for someone with a disability or nearing the end of life.
We recognise that many carers don't know about or receive much support. We now have a named Carer's Coordinator, Jennie Wilson, who has developed a pack of useful information and is also available for just a chat! She works alongside our doctors and nurses to ensure we recognise who are carers are, and offer support and signpost to appropriate services in a timely manner.

The attached is a list of services that your doctor is happy to complete for you. These are not under the NHS and therefore a fee is payable. The fee will be agreed with you beforehand. Payment will be required in advance and can be made by cheque or cash and a receipt will be given to you. Please allow a minimum of 3 weeks for completion - some may take longer depending on the level of information required.
Please hand in your form/letter/request to a member of our Patient Services Team along with your payment. You do not need to make a GP appointment for these requests.
